The risk profile, however, remains highly variable. A safari in Kruger National Park, the Okavango Delta, Victoria Falls, Zambia or Malawi presents a very different exposure level from a city break in Cape Town, Windhoek or a mountain retreat in Lesotho. Recent research also indicates that climate change is gradually shifting malaria hotspots eastward and southward on the continent, extending transmission seasons and reaching areas previously considered lower risk. For African travel professionals, this evolving picture reinforces the need for destination-specific, itinerary-driven advice.
In South Africa, malaria risk is concentrated in the north-east, particularly in Kruger National Park, the lower-altitude parts of Mpumalanga and Limpopo, portions of northern KwaZulu-Natal and border zones adjoining Mozambique and Zimbabwe. Transmission typically runs from September to May, placing October firmly within the active season. Cape Town, Johannesburg, Pretoria, the Garden Route and most of the Western and Eastern Cape remain malaria-free.

October's importance lies in the convergence of rising temperatures, early rains and peak tourism arrivals. Standing water expands mosquito breeding sites, while warmer conditions accelerate both mosquito development and parasite incubation. A common industry misconception is that a dry-looking landscape signals a low-risk destination, yet malaria transmission can persist even in relatively arid conditions.
The trade should also remember that symptoms can mimic other illnesses, including fever, chills, headache, muscle aches, nausea and fatigue. Plasmodium falciparum, responsible for most African malaria cases, can progress dangerously fast if treatment is delayed. Clients developing fever during or after travel to a risk area must inform medical professionals of their travel history, even if they took preventive medication.
